Becoming a Patient
To become a certified medical marijuana patient in the state of Connecticut, you must do the following:
Step 1
You must have one of the approved qualifying conditions:
For Adults, Debilitating Medical Conditions include:

- Positive Status for Human Immunodeficiency Virus or Acquired Immune Deficiency Syndrome (Effective 2012)
- Parkinson’s Disease (Effective 2012)
- Multiple Sclerosis (Effective 2012)
- Damage to the Nervous Tissue of the Spinal Cord with Objective Neurological Indication of Intractable Spasticity (Effective 2012)
- Epilepsy (Effective 2012)
- Cachexia (Effective 2012)
- Wasting Syndrome (Effective 2012)
- Crohn’s Disease (Effective 2012)
- Post-Traumatic Stress Disorder (Effective 2012)
- Sickle Cell Disease (Effective 2016)*
- Post Laminectomy Syndrome with Chronic Radiculopathy (Effective 2016)*
- Severe Psoriasis and Psoriatic Arthritis (Effective 2016)*
- Amyotrophic Lateral Sclerosis (Effective 2016)*

For Patients under 18, Debilitating Medical Conditions include:
- Cerebral Palsy (Effective 2016)

- Irreversible Spinal Cord Injury with Objective Neurological Indication of Intractable Spasticity (Effective 2016)
- Severe Epilepsy (Effective 2016)
- Terminal Illness Requiring End-Of-Life Care (Effective 2016)
- Uncontrolled Intractable Seizure Disorder (Effective 2016)
- Muscular Dystrophy (Effective 2018)*
- Osteogenesis Imperfecta (Effective 2018)*
Step 2
A patient may only register for a medical marijuana certificate if he or she is a Connecticut resident being treated for a debilitating medical condition by a Connecticut-licensed physician or advanced practice registered nurse certified with the Connecticut Medical Marijuana program.
Step 3
Once the certifying physician sends the appropriate online form to the Medical Marijuana Program office, you will receive an email within 2 days instructing you to create a DAS BIZNET Account so you can access the online registration system using the email address you provided to your physician. If you do not receive an email within 2 days, contact your physician’s office to confirm your email address and confirm the application was sent. Click here to create your DAS BIZNET account.
Step 4
After creating your DAS BIZNET account, you will be requested to upload the following documents:
- Proof of Identity (see examples)
- Proof of Connecticut residency (see examples)
- Note as of 7/1/23 – the state of CT has eliminated the $100 fee
You will be able to upload these documents and pay the fee when you submit your registration application online. If you pay by credit card, the charge on your statement will show the payee as “DCP Drug Control”.
Step 5
After you have uploaded all required documents and submitted your payment, please allow the State 30 business days to process your application.
Step 6
Once your application has been approved, you will receive an email with your temporary certificate that can be used at your designated dispensary.
