We are a Connecticut dispensary located in New Haven and Bridgeport. If you already have a valid Medical Marijuana card and are interested in transferring to Affinity Dispensary, it's never been easier!

Simply Stop By and Meet with One of our Cannabis Care Team Members!

1. Come on down to Affinity with your Medical Marijuana card and valid photo ID
2. Fill out a couple pages of paperwork, so we can get to know you! Or print the forms at home and save some time when you get here. (Intake Paperwork Form and Privacy Policy / HIPPA Form)
3. Once you are entered as one of our patients, you can come in and place an order

• If you know in advance that you are going to transfer to us, call us at (203) 542-7510 - Bridgeport or (203) 745-3823 - New Haven or email us at [email protected] and we can email you the paperwork to fill out prior to your first visit so it's one less thing to do when you get here! • After your initial visit there is no more paperwork involved, simply place a pre-order and pick it up when it's ready
