Do I qualify?
The first step is to identify if you have one of the many qualifying conditions. This can be done by referencing the master-list on our Becoming A Medical Patient page on our website or making an appointment with your registered primary care physician and/or qualifying medical provider.
What are some of the qualifying conditions?
- Complex Regional Pain Syndrome
- Autism Spectrum Disorder
- Chronic Pain
- Severe Rheumatoid Arthritis
- Epilepsy
- Cancer
Making an Appointment with a Provider:
Once you have identified your qualifying condition, making an appointment is seamless! Here at Affinity, we make this even easier. If you’d like to chat with a medical provider in person, come and see us on our provider days! We have various medical providers at both locations throughout the month to help YOU get your questions answered and make an appointment on the spot!
September Provider Days:
- Friday, September 6th from 2-5 (Bridgeport)
- Monday, September 16th from 4-6 (New Haven)
- Thursday, September 26th from 3-6 (Bridgeport)
- Friday, September 27th from 12-4 (Bridgeport)
Creating a DAS BIZNET Account:
Once you get approved by a physician, the next step is creating a DAS BIZNET account. This is required to register yourself as a medical patient in the state of CT. For directions click here.
You will need these supporting documents:
- Proof of Identity
- Proof of Connecticut residency
- NO MORE CT STATE FEE AS OF JULY 2023!
